You have two options for how to make a nomination.

 

*  If you prefer fill-in-the-blanks but want to do it by hand, you can print out the form.  Click HERE for a Nomination Form to print and instructions.

*  If you prefer to write your own narrative starting with a blank piece of paper, use the Nomination Outline.

The instructions for each option are slightly different because of the different formats.  Whichever way you choose, you will be requested to answer the same questions.
